About Constellation Community Living

Constellation Community Living (CCL) is a community‑rooted initiative that brings together individuals, families, direct support professionals, agencies, friends, neighbors, and allies who believe adults with IDD and autism should live fully and naturally in their own communities.

Our CCL Mission

CCL illuminates stars within the local community so each individual with IDD and autism can live the life they choose and thrive in their own home, neighborhood, and constellation.

We make the local community fertile ground for person‑centered living supports, so agencies and direct support professionals (DSPs) can succeed and individuals can flourish.

“CCL Stars” include each person, resource, and place in our constellation that is committed to creating inclusion, belonging, and compassion. A “constellation” is the network of relationships, supports, and local places that helps each person live well in this community.

Each person has their own constellation of support; together we form our larger constellation community.

We Work Locally

We work within a local community of four sister cities within a 5-mile radius. This is the scale where life actually happens — in the places, routines, and relationships people return to every day.

Our Constellation of families, agencies, DSPs, neighbors, and allies covers the northernmost cities in the San Andreas Regional Center area at the top of the South Bay: Mountain View at the center, Palo Alto to the north, Los Altos to the west, and Sunnyvale to the south. Our intention is to create a model for inclusive living that can be leveraged by others to develop constellations in their local geographies.
